“I found out that I needed an operation for a back problem which was causing severe pain to my left leg from the sciatic nerve. After speaking to my GP and realizing I was getting nowhere I decided to take the matter into my own hands as I could hardly walk. After speaking to private consultants and realizing that without private medical insurance I was not going to be seen to in this country without shelling out around £20,000 I decided to look elsewhere. I decided to look into having the operation privately in India and was fortunate to come across Mediescapes India. From my initial enquiry when I e-mailed my MRI results to Mediescapes India I was amazed ..more.”

SURGICAL PROCEDURES AND SPECIALITIES

MIOT Hospitals is internationally recognized for its outstanding comprehensive services. The surgical treatment provided to each patient is with the highest possible level of care. Being patient-centric means that we are always looking for ways to make procedures safer and more comfortable for patients and with better outcomes. As a result MIOT Hospitals has emerged as a key centre for innovations in surgical procedures and specialities like Keyhole Surgery, Pinhole Surgery, Computer Aided Joint Replacement, Endovascular Grafting and many more.

Keyhole Surgery : Imagine doing complex surgeries through an incision the size of a KEYHOLE.

In Laparoscopic or Keyhole Surgery a telescope is introduced into the body cavity through a 0.5cm opening. The area is viewed through a CCD video camera attached to the monitor. Surgeons can perform any type of surgery by viewing the monitor. Because of the smallness of the opening, this kind of surgery is called Keyhole Surgery, Minimal Access Surgery, or based on the equipment used, Laparoscopic or Endoscopic Surgery.

Small Incision, Minimal Pain and No Large Exposure - so chances of infection are almost nil. Keyhole surgery is cosmetically acceptable and allows quick recovery - You’re back in the gym after a major Laparoscopic Surgery in 24 hrs.

There are many advantages to the patient undergoing keyhole surgery when compared with an open procedure

- Reduced blood loss: reduces the chance of needing a blood transfusion.
- Smaller incision: reduces pain and shortens recovery time.
- Less pain: so less pain medication needed.
- Reduced infections: due to reduced exposure of internal organs to the possible external contaminants, risk of acquiring infections is reduced.
- Relatively small scar: Since the keyhole surgery involves a small opening it leaves relatively very small scar in the body.
- No cutting of muscles and tissue: So quicker scope of healing
- Though procedure times are usually slightly longer, hospital stay is less, and often with a same day discharge which leads to a faster return to the routine life.

Keyhole Surgery calls for extensive investment in probes and miniaturized equipment, and training of surgeons and surgical teams. But, it offers powerful benefits to patients that make operations less dreaded.

Pinhole Surgery : In the field of Interventional Radiology a whole range of procedures can be performed using imaging guidance, thereby giving patients a safe and viable option to conventional open surgery. MIOT took the lead in developing a Department of Interventional Radiology so that many complex surgeries could be converted to Pinhole Surgeries.

Pinhole Surgery is based on a simple thought. The human body is richly networked with arteries and veins that carry blood. So why not use them to access every part and every organ?

So, thought a puncture the size of a pinhole, and a catheter as thin as a hair, the pinhole surgeon with infinite care travels through the arteries to the affected area and delivers treatment directly to the site.

There is No Cutting, No Bleeding, and no need for Anaesthesia. Pinhole Surgeries improve outcomes, shorten recovery, reduce complications and open up new methods of diagnosis and treatment. Many of these pinhole procedures have replaced traditional open surgeries performed in a hospital. Pinhole surgery approach can be used in procedures involved with: Heart, Kidney, Tumour, Stroke, Eye and many more.

Joint Replacement and Revision Surgery (Hip and Knee Replacement) : MIOT gives a beginning, almost where hope has ended. A pioneer in Joint Replacement Surgery in India, MIOT Hospitals has specialized in this area for over three decades and has to its credit rich experience in replacement surgery of hip, knee, shoulder and elbow.

Joint Replacement Surgery is undoubtedly the most significant and scientific development in this century for it helps the patient to bounce back to normalcy within a span of ten days.

Revision surgery demands a high level of planning, customized component design and the ability to overcome bone deficiency caused by wear and tear. Not many surgeons in the world can perform revision surgery, and MIOT is one of the few places with the technology and expertise in this field.

As the country’s foremost centre for Joint Replacement Surgery we have performed 23,000 Hip and Knee replacement surgeries. This gave us special insights on how a hip or knee replacement could last a lifetime. The MIOT Global Centre for the Ideal Joint was launched to collaborate with specialities the world over, to develop an optimum implant for Indian anatomies. One that can last a lifetime.

Limb Lengthening : The limb lengthening process works by gradually growing new bone and soft tissues (skin, muscle, nerves, blood vessels, etc). This new growth is called tissue regeneration. Bone and soft tissue regenerate when they are distracted (pulled apart) at a very slow rate of approximately 1 mm per day. If the rate of distraction is slow rate of approximately 1 mm per day. If the rate of distraction is that are being pulled apart and soft tissues, such as muscle, may experience contracture (get too tight) or nerves may become paralyzed. If the rate of distraction is too slow, premature consolidation may occur (the bone may consolidate too soon), preventing the lengthening device from further pulling it apart. The lengthening devices used could be external fixators or those that are fully implanted inside the bone. There are two phases of lengthening until the bone is fully healed: the distraction phase and the consolidation phase. The former is the lengthening phase. After the desired length is obtained, the newly regenerated bone is still very weak because of lack of calcium within it. The hardening and calcification of this new bone is called the consolidation phase.

Re-Implementation of Severed Fingers & Limbs : This specialized field of Microsurgery is a virtual boon to accident victims. It is in fact recreation of God's creation for this procedure makes it technically possible to re-implant a severed part of the body, especially the hands and legs. The success of this procedure largely depends on timely arrival of the patients without much loss of time after the accident.

The Microsurgery team at MIOT comprises Hand and Microvascular Surgeons with vast experience in this critical procedure.

They form the re-implantation team, which plunges into action within 15 minutes of the arrival of an emergency patient. In fact, their experience with many accident victims in this industrial belt has motivated the team to carry out awareness campaigns amidst factory workers. The latter are educated about the right method of handling the injured worker before rushing him to MIOT.

Computer Aided Joint Replacement Surgery : MIOT is a pioneer in Computer Aided Joint Replacement Surgery which helps in ligament balancing and placement of the prosthesis with zero error.

Normally the problem with artificial joint is the long term survival of the joint. Like any other artificial component, artificial joints have a life, some last 20 to 25 years. To make them permanent, scientists the world over have researched the material to make them survive longer, from high density polyethylene to ceramics, then to titanium and then reverted back to steel. Metal on metal. It did not solve the problem. Also they switched over from cemented to uncemented joints where bonding is obtained by porous coating. This has enhanced the life of the joint by a few years.

Now it is the considered opinion of scientists that if the implant can be implanted with correct anatomical alignment, with zero error, we can improve on the longevity of the artificial joint. The computer is also monitoring every step of the Surgeon, constantly guiding him to perform the surgery with Zero Error. This we think is the greatest achievement of modern surgery.

The important point here is that just as any persons fingerprint is different, every person’s anatomical knee axis is slightly different.

Benefits of Computer Navigated Surgery ~

- This surgery will ensure the patients total knee components will be implanted in the original anatomical axis with 0 degree or near 0 degree error.
- It will lessen the time of the operation because of 5 cuts on the femur are made with one instrument jig.
- Because no intramedullary rods are used to position the femoral instrument, the incidence of fatal fat embolism will be less.
- Accurately positioned knees will be constantly possible with every patient. With anatomically implanted components with regards to the axis of the limb, component wear should be minimized and hence the life span of the knee should definitely increase.

Neuro Surgical Care : The Department of Neurosurgery at MIOT Hospitals performs around 200 major neurosurgical procedures every year with a success rate of above 98% and zero percent infection rate.

Procedures Performed ~

- Brain Cancer: Microsurgical, Endoscopic and Stereotactic excision of brain tumors like Gliomas, Meningiomas, Skull base tumors, Posterior fossa tumors, Pituitary Tumors, Intraventricular tumors.
- Brain Hemorrhage: Clipping of the Aneurysm, Evacuation of Intracerebral Hemorrhages, Surgery for Stroke. Aneurysms are coiled by the interventional radiologist.
- Functional Neurosurgery: Various types of surgery for chronic intractable pain including cancer pain.
- Trigeminal Neuralgia: Microvascular decompression, Retrogasserian ganglion glycerol injection.
- Spine lesions: Microdiscectomies for disc prolapse, excision of spinal cord tumours, repair of spinal birth defects. Artificial Cervical Disc replacement has been recently started.
- Craniovertebral junction anomalies like Basilar invagination, Atlanto-arial; dislocation, Dens fracture and Arnold Chiari Malformation are being operated. Various stabilization procedures for CV Junction are also being performed.
- Head injuries and its sequel are being managed.

Endovascular Grafting (Aneurysm) : One of the most challenging areas for a surgeon is aortic aneurysm surgery. Our Centre is one of the preferred referral centres for Aortic Aneurysm repair in India.

Aneurysm is a bulge or swelling in the artery that has been weakened by disease or injury. The danger is that it may rupture causing internal bleeding that could be fatal. It is a condition where surgery is very complex and difficult. In certain procedures the patient’s blood temperature is brought down to 15degree and blood circulation is stopped. On reaching this stage the procedure has to be performed within 45 minutes.

Endovascular Grafting – A new technique without Open Surgery

Endovascular Stenting is done only in very few centers in the world and METAA (MIOT Endovascular Centre for Aortic Aneurysm) at MIOT Hospitals in one such centre.

Beating Heart Surgery : Beating heart surgery is a revolutionary technique in bypass surgery.

The procedure offers distinct advantages ~

- No requirement of blood transfusion
- Up to 30% savings in costs over conventional bypass surgery
- Minimal stay in ICU and hospital
- Minimized chances of infection
- Faster rate of recovery

This procedure is a boon to patients classified as high risk, due to old age related lung diseases, kidney failure, poor pumping capacity of the heart, etc.

Beating Heart Surgery can be performed only by competent surgeons supported by an experienced team. MIOT is one of the leading and best hospitals for undergoing Beating Heart Surgery where this surgery is performed in 98% of the cases which comes to us.

Valve Repair : Heart valves control the direction of blood between the atria, the two upper chambers and the ventricles, the two lower chambers of the heart. The atria receive blood from outside the heart and pump it into the ventricles and the ventricles pump blood away from the heart. The four heart valves namely, tricuspid, mitral, pulmonary and aortic perform the critical function of purifying impure blood and circulating it to all the vital organs.

Heart valve leakage results from malfunction of one or more valves. In such a condition the heart has to work harder to deliver oxygen rich blood around the body. The overworked heart may begin to fail, causing shortness of breath, pain, tiredness and build up of fluid in the body. If this happens the valve may have to be replaced or repaired. If the vale is replaced, patients have to take life long blood thinning medications.

This is not the case if the valve is repaired. The surgical team at MIOT has extensive experience in mitral valve repair – more than 300 operations with 99% success rate. MIOT is one of the few hospitals in the country performing this surgery.

Coronary Angioplasty : It is the non-surgical mode of treating arterial / venous blockages using balloon and other related novel devices to restore blood flow in the affected blood vessels. Angioplasty is a much quicker and simpler procedure than Bypass surgery, wherein the patient is ambulant soon after the surgery and can resume routine activities in a day.

The procedure involves an arterial puncture either in the arm or conventionally at the groin. Due to recurrence of blockage within few years of bypass surgery, angioplasty evolved as an alternative choice. Coronary angioplasty done during an acute myocardial infarction (primary angioplasty) is currently the best treatment for heart attack. With unrelenting research, science has shown the way to the highly promising technology of using sirolimus coated stents. The restenosis rate post this angioplasty procedure is zero, thus making it the best treatment available for myocardial infarction.

In Europe's leading centers, this procedure is done through the arm which makes it possible to conduct invasive tests and interventions as an out-patient day care service for coronary peripheral arterial diseases. The MIOT cardiac care team has to its credit rich experience making MIOT one of the few hospitals in the country where coronary angioplasty is performed using the latest technology.
