The Madras Institute of Orthopaedics and Traumatology (MIOT), had its
genesis as an independent wing of Vijaya Hospital, where Padmashree
Prof. Dr.P.V.A. Mohandas and his team performed many a surgical
impossibility in the field of Orthopaedics. Subsequently, his
international travel exposed him to the highly impressive standards of
hygiene and competence of medical services in the West He grew
obsessed with an overpowering desire to provide similar standards of

Keyhole Surgery
Keyhole surgery is possible due to the latest miniaturized technology
attached to scopes like the bronchoscope featured here.
Through a small hole a Telescope with video camera is introduced into
the abdomen. Images from the camera are projected onto the monitor.
Surgeons can do any type of surgery by viewing the monitor with
additional 0.5cm holes in the abdomen.

Joint Replacement & Revision Surgery
"Management of Arthritis" is a specialty of MIOT Hospital. In
Arthritis all the movements in the Joints are restricted by pain.
Beyond a certain stage, pain killers, injections, massage or any other
form of healing may not be adequate. Once restriction of movements
occurs in the joints, there is need for replacement.

experience in replacement surgery of hip, knee, shoulder and elbow.
Joint Replacement Surgery is undoubtedly the most significant and
scientific development in this century for it helps the patient to
bounce back to normalcy within a span of ten days.
Revision surgery demands a high level of planning, customized
component design and the ability to overcome bone deficiency caused by
wear and tear. Not many surgeons in the world can perform revision
surgery, and MIOT is one of the few places with the technology and
expertise in this field.
Knee Replacement
Surgery of the hip and knee may be required for a number of reasons -
degeneration, injury, disease or simple ageing of the joint. In these
cases, an artificial joint is inserted. But even artificial joints
have a limited life and need to be replaced after around 15 years.
Artificial joints cause erosion of the surrounding bone and tissue,
which poses a challenge for the insertion of another joint.
Actually, the doctor tries and saves as much of the patient's bones,
ligaments and muscles as possible. The new knee replaces only the
rough cartilage at the ends of the bones with smooth metal and plastic
coverings. This helps ease the pain caused by the bones rubbing during
activity or while in bed at night.
Physical therapy will be very important after surgery to regain the
motion and strength in the new knee. Pain medication will be provided
to help make these tasks easier. Over the next few days normalcy
gradually returns.
Hip Replacement
Once there is trouble experienced in the hip area, it becomes hard to
walk or stand. Gradually a person's ability to enjoy daily activities
suffers. For most patients, joint replacement surgery provides relief
from pain. Hip replacement surgery has been performed for decades to
help relieve pain and increase mobility for people with severe
arthritis.
Doctors replace diseased hips with implants made of metal or medical
grade plastic. Today, with advances in implant design and surgical
techniques, doctors are focusing on ways to help reduce recovery time
and improve comfort after the operation. World class equipment and
surgical expertise in this area at MIOT make the recovery time of
patients faster and less time off their feet after surgery.
Limb Lengthening
The limb lengthening process works by gradually growing new bone and
soft tissues (skin, muscle, nerves, blood vessels, etc). This new
growth is called tissue regeneration. Bone and soft tissue regenerate
when they are distracted (pulled apart) at a very slow rate of
approximately 1 mm per day. If the rate of distraction is faster than
this, bone may fail to form between the two ends of the bone that are
being pulled apart and soft tissues, such as muscle, may experience
contracture (get too tight) or nerves may become paralyzed. If the
rate of distraction is too slow, premature consolidation may occur
(the bone may consolidate too soon), preventing the lengthening device
from further pulling it apart. The lengthening devices used could be
external fixators or those that are fully implanted inside the bone.
There are two phases of lengthening until the bone is fully healed :
the distraction phase and the consolidation phase. The former is the
lengthening phase. After the desired length is obtained, the newly
regenerated bone is still very weak because of lack of calcium within
it. The hardening and calcification of this new bone is called the
consolidation phase.

Computer Navigated Joint Replacement Surgery
The problem is the long term survival of the joint. Like
any other artificial component, artificial joints have a life, some
last 20 to 25 years. To make them permanent, scientists the world over
have researched the material to make them survive longer, from high
density polyethylene to ceramics, then to titanium and then reverted
back to steel. Metal on metal. It did not solve the problem. We also
changed the design of the joints, without much success.
We switched over from cemented to uncemented joints where bonding is
obtained by porous coating. This has enhanced the life of the joint by
a few years.
Now it is the considered opinion of scientists that if the implant can
be implanted with correct anatomical alignment, with zero error, we
can improve on the longevity of the artificial joint. The computer is
also monitoring every step of the Surgeon, constantly guiding him to
perform the surgery with Zero Error. This we think is the greatest
achievement of modern surgery.
The important point here is that just as any persons fingerprint is
different, Every persons anatomical knee axis is slightly different.
Benefits of Computer Navigated Surgery
This surgery will ensure the patients total knee components will be
implanted in the original anatomical axis with 0 degree or near 0
degree error.
It will lessen the time of the operation because of 5 cuts on the
femur are made with one instrument jig.
Because no intramedullary rods are used to position the femoral
instrument, the incidence of fatal fat embolism will be less.
Accurately positioned knees will be constantly possible with every
patient. With anatomically implanted components with regards to the
axis of the limb, component wear should be minimized and hence the
life span of the knee should definitely increase.
